I have a copy of "Epochs in American History", by Edward S. Ellis, A.M. The title page also shows "American History Series, No. 2." It was published by A. Flanagan Company, Chicago, copyright 1896 by A. Flanagan.
Its last page (containing pages 191 and 192) is missing. Its page 190 ends with these two lines:
