My personal practice is to use curly braces for page numbers and square brackets for footnote numbers. I include page numbers in an etext only if the book has internal references of some kind, e.g. footnotes that refer to specific pages, an index, or a table of contents that's sufficiently detailed as to function as an index. I number only the first page of an index, since I've never seen one with references to elsewhere in itself. Two-column indexes are rendered as single-column. Is there any suggestion what a formatted text-only book that retains page numbers should look like? Is it reasonable to just sprinkle them into the text, maybe something like this:
