I checked several of my RST-based projects with Firefox's epub plug-in, and all show the source book's cover artwork. 
 
My question would be, why would you *not* include the cover art, as you would any other illustration, in the main body of the RST file?

I don't know about RST, but if you want to include a cover image with an HTML e-book, it has to be a jpg, not a png. I'm guessing it would probably be the same in RST, since I think it's a limitation of either epub or mobi or some readers or a combination of these.
