Greg, it was decided early in DP's discussions of RST that the submitter would be responsible for the metadata block, since they were already familiar with the book's basic information--title, author, language, publication date, etc. The WWers edit the PG:Id field and the PG:Release field, making only any necessary corrections to the other fields. For book number: we assign this when it is posted. The only time we assign in advance is when there are dependencies (such as, two volumes that need to reference each other). You don't need to do any of the header/footer/metadata, since that is all added at posting time.
