Use a microfilm reader that can do photocopies and then scan the copies. Sometimes this works good and sometimes it doesn't. It always includes the added expense of doing the copies. I thought microfilm pretty well was 35mm film. The scan area of my film scanner is still only the middle 24mm (where a 35mm camera puts its frames) though, so it would not work. A medium format scanner would work okay. You'd still have to cut up the film though. You might be able to use a digital camera to photograph the pages on a microfilm reader's screen. You'd need to disable the flash and use a tri-pod most likely. Another option might be a slide/film copier attachment in a digital camera. Expensive setup that. Finally, a bit of googling reveals that there are companies that provide digitization of microfilm as a service. It may cost a bit, but probably not that much, to have it scanned.
