Thanks to Jeroen Hellingman for updating this eBook with an improved processing chain for mathematical works.  

For the curious: The processing chain includes creation of an epub, which we do not typically include (they are instead generated automatically from HTML).  Also for the curious: Equations and other notations (there are many!) are now faithfully and consistently produced using mathematical markup.
